Webinar Summary:  In this session you will hear about the value of a whole person care approach to assessing and caring for individuals whose health is affected by a combination of physical, mental and social issues. Supporting team collaboration and engaging the individual as a contributor and owner of their care plan is key. A population health strategy that incorporates this broader perspective can effectively identify and address fundamental issues affecting the well-being of each of your tribal members.
